To determine eligibility for services, the student is responsible for providing documentation of his/her disability.

Disability documentation must be within the last three years, and must include the educational limitations specific to the student. The documentation must be signed by a licensed health care professional. Examples of licensed professionals include: physicians, psychologists, audiologists, licensed clinical social workers, learning disability specialists and vocational rehabilitation counselors. (We cannot accept verification from a chiropractor or holistic health practitioner). If the student does not have documentation of disability, DSPS can send a release of information form to the student’s health care provider requesting this information. If a student needs to request documentation of disability, he/she must provide the name, address, telephone and/or fax numbers of the physician or other appropriate licensed professional
