Step 3: The DSPS INTAKE INTERVIEW and Determining Eligibility for Services
The DSPS intake interview is the initial meeting between the student and a DSPS counselor. The intake is approximately one hour, and during that time the student and counselor will discuss topics such as: educational goals, academic history, disability information, educational limitations, and possible DSPS services and accommodations.

To determine eligibility for services, the student is responsible for providing documentation of his/her disability.

Disability documentation must be within the last three years, and must include the educational limitations specific to the student. The documentation must be signed by a licensed health care professional. Examples of licensed professionals include: physicians, psychologists, audiologists, licensed clinical social workers, learning disability specialists and vocational rehabilitation counselors. (We cannot accept verification from a chiropractor or holistic health practitioner). If the student does  not have documentation of disability, DSPS can send a release of information form to the student’s health care provider requesting this information. If a student needs to request documentation of disability, he/she must provide the name, address, telephone and/or fax numbers of the physician or other appropriate licensed professional
